Vision Insurance
To search for providers within Humana's National POS OpenAccess network, follow these steps:

Input zip code

Select "Insurance through your employer"

Scroll down and select "National POS -OpenAccess"

Search the doctor/facility.
Life Insurance
To search for providers within Medical Mutual's SuperMed PPO network, follow these steps:

Select "Group" then click "Next"

Select "Medical"

Choose "SuperMed PPO"

Enter the city/zip code

To search for providers within United Healthcare's Choice Plus network, follow these steps:

Scroll down and select "Choice Plus"

Depending on your plan, verify if they accept the "OH DCPG PPO" or "OH DCPG DHMO."

Superior Dental Instructions

VSP Vision Instructions
To search for providers within VSP's network, follow these steps:

Enter office name or doctor's name

Select a state

EyeMed Vision Instructions
To search for providers within EyeMed's Insight network, follow these steps:

Select the "Insight" network
